Care Coordination 


Uplift, Inc. provides conflict-free care coordination services for adults who receive Section 18 Brain Injury Waiver funding.

Person’s receiving this service can expect to work with their care coordinator in a proactive, person centered approach to identify their needs, desires, goals and develop a comprehensive plan to work towards success in reaching these goals.

Services provided include:

  • Gaining access to needed waiver services (home supports, technology assessment, work supports etc.)

  • Gaining access to medical, social and educational services

  • Coordinating  the acquisition or retention of natural supports

  • Monitoring the implementation of the care plan

  • Monitoring of the health, safety and welfare of the member to include a face to face visit at a minimum of every 30 days.


Services are provided in Kennebec, Somerset, Androscoggin, and some parts of Knox County.
